Insurance
We will work with you, your insurance plan and your budget. Fees are based on care and judgment - not only on the cost of your treatment, but also on the time it takes to provide full dental care. You may make a partial payment at the time of treatment and pay the balance in increments; time payments are subject to interest charges. Bring all insurance information and a claim form with you. The staff will complete the dental portion of the form. If your carrier requires a predetermination or prior authorization, we will submit a treatment plan to the insurance company for review. If you direct the company to pay its share of the cost directly to the office, you will receive credit for this amount and may pay your portion at the time of service or pay in installments. Insurance benefits vary greatly. Some plans cover only a portion of the complete dental treatment we offer. They exclude certain types of services, such as temperomandibular joint therapy (TMJ) and cosmetic dentistry. Benefits may be based on a chart or schedule of fees arbitrarily developed by insurance companies. So if your plan covers 80 percent of the cost of dental treatment, it could mean 80 percent of the fee determined by the company, not 80 percent of the actual fee. Familiarize yourself with your particular policy. Again, we will work with you, your insurance plan and your budget. |
