• Based on a thorough examination, the dentist determines the range of choices available to the patient for meeting his or her dental care needs;
• After conferring with the dentist, the patient selects the most appropriate options for his or her circumstances;
• A continuing care program is planned that incorporates the selected options, and involves appropriate levels of participation by both patient and dentist.
